ACH dispute

To report ACH disputes or fraud with transaction codes ‘CCD’ or ‘CTX’, please fill out the form linked below. For all other ACH disputes, please contact United’s Member Service Center at (888) 982-1400 as soon as possible. You will need the following items:

  • Current date
  • Account Number
    Enter the full account number that received the unauthorized transaction
  • Business Name
    Name on the account
  • Authorized Signer
    Name of authorized signer on account
  • Amount of Debit
    Exact amount of unauthorized transaction
  • Date of Debit
    Date unauthorized transaction posted to account. Can only be previous and current business day
  • Party Debiting the Account
    The debiting name exactly as it shows on your account

This form must be received prior to 3:00 pm ET no later than the business day after the transaction posted in order to be returned.

Once you complete the form:

  • Print the document
  • Have the authorized signer sign and date the form
  • send to United before 3:00 pm ET to bob@unitedfcu.com or by fax at (269) 982-7939

Disputes or check fraud, must be reported within 48 hours of clearing the account. To report check disputes or fraud, contact United’s Member Service Center at (888) 982-1400 as soon as possible.
